How do I submit my letter of diagnosis?

A letter or document from your medical provider verifying your cancer diagnosis is required as part of your application. The document can be:

1. Uploaded to the application (by scanning or uploading a photo)
2. Emailed to admin@sanbornfoundation.org
3. Mailed to:
